Primary Care Services
Your primary care doctor manages your overall health and wellness, including routine checkups, screenings, and preventive care. He or she is the person you call when you have a cold, notice a change in your health, or need an annual physical.
Think of your primary care doctor as someone who specializes in you. Your doctor gets to know you and your health history to help you maintain good health. He or she coordinates your healthcare with specialists (such as cardiologists, orthopedic surgeons or others) and maintains your prescriptions to keep you safe.

Transitional Care Clinic
