Implementation of a Pharmacist-led Comprehensive Medication Management Service at the UMMC Midtown Health Center

Project Overview
Patients with uncontrolled hypertension are at risk of more acute hospitalization and ED visits which will result in a higher total cost compared to those with controlled hypertension. Pharmacist-delivered Comprehensive Medication Management (CMM) service is an evidence-based strategy recommended by the CDC to improve cardiovascular health outcomes for patients with hypertension.
Our goal is to implement CMM delivered by Pharmacists in a collaborative practice with physicians at the University of Maryland Midtown Health Center (UM MHC) for patients with hypertension. Utilizing both telehealth and face-to-face care delivery models, pharmacists will optimize drug therapy, improve medication adherence, identify, and address barriers to achieving targeted blood pressure goals. Patients enrolled in this service will have access to a home blood pressure monitoring device and other adherence-supporting tools.
