Please complete this form to request graphic design, writing, social media, and creative support services from the UM Capital Region Health Marketing and Communications team.

For planning purposes, please allow a minimum of two weeks to receive your first proof for review. We will always do our best to meet your deadlines and keep you informed of any necessary changes. Following submission of this form, you will be contacted by a team member to confirm receipt of your project request and due date. If you have a rush request requiring immediate attention, please contact UMCapitalCommunications@umm.edu and add "Rush" to the email subject line. While we cannot guarantee we can honor your request, we will discuss and do our best.
