Before beginning work at the University of Maryland Medical Center, every employee must schedule or complete (if remote) a pre-placement health evaluation with Employee Health Services (EHS). The pre-placement exam can be completed as early as 60 days prior to the scheduled start date. If you require an onsite evaluation, please do not wait to schedule your onsite visit as these appointments fill up quickly.
New this year, EHS will be using an Electronic Medical Record called Cority. This means that all of the forms and questionnaires can be completed electronically. In order for EHS to create an account for the resident/fellow within Cority, new residents and fellows should send an email to employee_health@umm.edu and include the following:
- Photo ID (should include last name, first name and DOB)
- Phone number
- Personal email address
- Last 6 of SSN
- Program name
- Role (such as PGY 1 or Fellow)
- Start date
EHS will set up an account for the resident/fellow based on this demographic data. Once the resident/fellow has an account set up within Cority, EHS will inform the resident/fellow that they will be able to log in with a username (personal email) and password (see tip sheet) and will be able to access myCority in order to complete all items as described in the Welcome email from Employee Health. Please remind your residents and fellows to complete all questionnaires within Cority such as health questionnaires, MD note if required, immunization record, TB screening record prior to their scheduled appointment if they are planning to come in for an onsite appointment.
Deadline for submission of questionnaires to EHS:
April 15, 2025 - Deadline for residents to return health evaluation paperwork if starting in June.
May 1, 2025 - Deadline for residents/fellows to return health evaluation paperwork if starting in July.
June 1, 2025 - Deadline for residents/fellows to return health evaluation paperwork if starting in August.
