Student Welcome Forms
We are looking forward to your arrival for your clinical affiliation within the Department of Rehabilitation Services at the University of Maryland Medical Center.
All students are processed through volunteer services. Prior to your clinical experience, paperwork must be submitted to volunteer services and to rehabilitation services. Please follow the instructions below for both.
Required Processing Through Volunteer Services
- Go to the volunteer services website. There, you will find links to the volunteer services application and the health clearance and background investigation forms.
- Download and fill out the health clearance and background investigation forms.
- Click on the volunteer services application. When filling out the online form, be sure to answer "Yes" to the question, "Do you have a confirmed placement?" (This will cue volunteer services to create your UMMS U account, which is where your Non-Employee orientation modules and Epic modules will be assigned.)
- Closely read the application, including the portion regarding required documentation of titers, immunization records, flu vaccination, COVID-19 vaccination, PPD, etc. (as outlined on the Health Clearance Form). Regarding TB testing, UMMC requires that within 90 days prior to the start of the clinical experience, you provide documentation of either: 2 separate PPD (skin placement) tests OR 1 TB blood test.
- Upload the health clearance and background investigation forms at the bottom of the online application form, and then submit.
- Once the application is submitted, please email your rehab services contact (listed at the bottom of this page) to confirm and submit the additional paperwork outlined below.

Of note, for each of the above forms, please check off "Send me a copy of my responses" and enter the email address(es) of the appropriate coordinator(s) for your school/discipline (see bottom of page).
