Insurance

Before your initial evaluation, we will verify with your insurance company if physical therapy (PT) is covered in your policy. If so, you will be responsible for co-pays at the time of services as well as co-insurances and deductibles.

If your insurance does not cover PT, you will responsible for full payment.

We accept many insurance plans, including most managed care plans. If you have questions about your insurance, please call 301-609-5494.

All patients are required to have a written prescription from their referring doctor, including diagnosis and services requested.

What to Bring to Your First Appointment

• Referring doctor's prescription
• Form of identification
• Primary doctor referral (if required by insurance company)
• Copies of medical reports (MRI, surgical report, etc.)
• Completed new patient paperwork available under Evaluation Forms

Please wear loose fitting clothing for comfort and mobility.

Evaluation Forms

Print, complete and bring the required forms to your initial evaluation. If you are unable to complete the new patient paperwork ahead of time, please arrive 30 minutes before your appointment to fill it out in the office.
