Release of Health Information

The Health Information Management Department at UM Charles Regional Medical Center maintains hospital medical records for patients treated at the facility. All patient health care information at UM CRMC is confidential. Therefore, medical records cannot be released to any person without the authorization of the patient or the patient’s legally authorized representative (unless authorized by law) and a picture ID.

Requesting Medical Records

All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, the parents or guardian must sign the authorization.

HealthPort medical records requests are processed through a contracted company with UM Charles Regional Medical Center. If processed through HealthPort, you will be contacted by them for payment. Records are mailed upon receipt of payment – patients should allow 10 business days to process the request. All records are mailed via U.S. Mail to the address specified on your request.

Birth Certificates

The Health Information Management Department is responsible for the submission of a birth certificate worksheet to Department of Maryland Vital Records within 72 hours for all live-born infants delivered at or in route to UM Charles Regional Medical Center. Department of Maryland Vital Records is solely responsible for distributing copies to qualified individuals.
