Effective Tuesday, December 5, 2023, at 4 pm, all UMMS team members, volunteers, partners and Medical Staff must have received the flu vaccination or have an approved exemption.

UMMS expects all team members to be vaccinated against the flu. In accordance with Federal and State law, UMMS provides exceptions for staff members who:

  • Have a medical reason or contraindication
  • Have a religious objection based on their own sincerely held religious beliefs

Staff members who do not have access to a computer should contact their local Employee Health or Human Resources department.

Prior Approved Exemptions

If you have an approved permanent exemption from a prior year, you may not need to resubmit an exemption form.

Team members employed by the entities below DO NOT need to resubmit their documentation

  • UMMS Shared Services
  • University of Maryland Medical Center – Downtown Campus
  • University of Maryland Medical Center – Midtown Campus
  • University of Maryland School of Medicine Clinical Faculty
  • UM Baltimore Washington Medical Center
  • UM Capital Region Health
  • UM Rehabilitation and Orthopaedic Institute
  • UM Shore Regional Health
  • UM St. Joseph Medical Center
  • UM Upper Chesapeake Health (permanent Medical Exemptions require an updated form but not a new Medical Provider Note)

Team members employed by the entities below MUST RESUBMIT their documentation

  • UM Charles Regional Medical Center 

Forms must be submitted, along with the necessary documentation, by Tuesday, November 7, 2023. We recommend submitting your request as soon as possible. Submission is not a guarantee of approval.


Medical Reasons

If a team member has a medical reason that would prevent them from being vaccinated, such as an allergy to the flu vaccine, the staff member must complete both steps below.

  1. Have their medical provider attest to the medical reason the staff member cannot receive the flu vaccine. The medical provider will need to complete the Flu Vaccination Medical Exemption form (PDF version | Word document).
  2. Fill out the online form below and upload the medical documentation from the treating medical provider.

Religious Reasons

To seek an exemption on religious grounds, a staff member must attest that they have a sincerely held religious belief that would prevent them from receiving the flu vaccine. The staff member must complete both steps below.

  1. Provide supporting documentation of the staff member's sincerely held religious belief using one of the following options.
    • A statement from the staff member using the Statement of Religious Objection to Influenza Vaccine form (PDF version | Word document), OR;
    • A letter from a member of the clergy attesting to the staff member's sincerely held religious beliefs. Letters from clergy must be printed on letterhead.
  2. Fill out the online form below and upload the completed Statement of Religious Objection to Influenza Vaccine form.
