Effective Tuesday, December 5, 2023, at 4 pm, all UMMS team members, volunteers, partners and Medical Staff must have received the flu vaccination or have an approved exemption.

Staff members who do not have access to a computer should contact their local Employee Health or Human Resources department.


Vaccinated at an UMMS Vaccine Location?

  • If you are an UMMS employee who received your flu shot at the UMMS hospital where you are employed, your hospital will have your vaccination status on file. No further action is required.
  • If you are an UMMS team member who is employed at one hospital but received your flu shot from another UMMS hospital, you will need to submit your proof of vaccination to your employer hospital.
  • For Corporate Shared Services employees only: If you received your flu shot from either the corporate employee health (EH) department or the UMMC Downtown Campus EH department, your vaccination status will be on file. If you received your flu vaccination from any other UMMS hospital, then you will need to submit your proof of vaccination to corporate employee health (EH) via the "Submit Your Documentation" button below.

Vaccinated at Another Location?

If you are an UMMS employee who received your flu shot at another location, such as a pharmacy, your primary care physician, your local health department or UM Urgent Care, please submit your vaccine documentation.

Note that if you work at one UMMS hospital, but received the flu shot at another UMMS hospital, you'll also need to submit your documentation via this form to the specific hospital where you are employed.
