Patient care relies on support from all areas of a person's life, including family, friends, hospital staff and medical providers.
We encourage family participation and are dedicated to educating and supporting family members whose lives are impacted by their loved one's medical needs. The patient care team holds regular family conferences to involve them in the rehabilitation process and to prepare them for meeting the patient's discharge needs.Ongoing patient and family support is available through the Stroke Club's monthly meetings which provides education and resource information in a social forum.
The Role of the Case Manager
The case manager is the link between the family and the members of the patient's care team. The case manager keeps family informed about patient progress and helps them make discharge plans and preparations. In addition, the case manager serves as the point of contact for insurance issues, working closely with the family and the insurer to identify and address any potential problems.
Outpatient Follow-Up Care
Upon discharge, patients participate in our follow-up recovery program, which promotes a continuation of progress toward independence. The Outpatient Follow-up Program at University of Maryland Rehabilitation and Orthopaedic Institute may include the following:
- Medical evaluation and treatment
- Occupational therapy
- Physical therapy
- Speech-language therapy
- Rehabilitation nursing
- Recreational therapy
- Referral for vocational rehabilitation.
