Ordering Instructions

Order forms can be picked up in the Translational Laboratory, or call 410-328-3914 to have us mail or fax them to you. Make certain that all forms are completed. It is important to provide all information requested to ensure that you are notified as soon as your order is processed. Information provided must be legible.
All orders require an account number and signature of the principal investigator or authorized designee. Incomplete orders will be returned. Orders forms can be faxed or e-mailed; bring the appropriate completed forms when you submit your samples.
The Translational Laboratory does accept orders from customers outside of the University of Maryland system. However, outside orders will require an additional institutional overhead. We can accept either purchase orders or credit card orders. Please contact us at 410-328-3914 prior to submitting orders to find out about our policies regarding ordering and payment.

Clinical Trial-Related Work Orders:
Clinical Trials With Sponsored Correlative Endpoint Assessments (e.g., NCI TRI funding):
The Translational Laboratory must be involved in the budget development for such trials and, if funds are approved, will collect and process samples according to a Standard Operating Procedure (SOP) that will be generated for the particular trial. We will invoice the study sponsor either quarterly or after a cohort of three (3) patients is processed.
Clinical Trials Without Sponsored Correlative Endpoint Assessments:
Correlative studies for non-sponsored trials will be considered as a custom work order. A "Pharmacodynamics Endpoint Assessment Custom Services" order form must then be completed. Order forms can be picked up in the Translational Laboratory, or call 410-328-3914 to have us mail or fax the form to you. Make certain that all forms are completed. It is important to provide all information requested to ensure that you are notified as soon as your order is processed. Information provided must be legible.
