ChoiceOne partners with the University of Maryland St. Joseph Medical Center (UM SJMC) to offer integrated, coordinated care with UM SJMC providers, as well as set up referrals to in-network physicians when needed.
During a patient’s visit, ChoiceOne team members work directly with a Patient Care Coordinator at UM SJMC to schedule follow-up appointments with a primary care provider at UM SJMC or individual specialist offices.
Patients leave ChoiceOne with peace of mind knowing they were well-cared for during their initial visit and will be taken care of at any subsequent provider visits.
