Our Center for Diabetes Education, located in UM Charles Regional Medical Center, holds classes, one-on-one counseling, nutrition therapy and more for diabetes patients. Learn more about our diabetes center and how it can help you live a healthy, active life with diabetes.

Hours

We're open five days a week:

• Monday, Wednesday and Thursday: 8 am - 4:30 pm
• Tuesday: 10 am - 5:30 pm
• Friday: 8 am - 4 pm

You need a referral form from your doctor before you make an appointment with the diabetes center, except to attend the support groups. Call us with any referral or appointment questions.

What to Bring to Your Appointment

Please arrive about 20 minutes early to the appointment so that you have time to complete your paperwork at Patient Registration.

Bring the following:

• Your insurance card
• A list of medicines
• Referral form (unless the doctor has faxed it to us)
• Blood sugar meter (if you have one)
• Blood sugar record
