For Adults

The adult Programs of Assertive Community Treatment (PACT) team is a program intended to meet the mental health and case management needs of adults, ages 18 and older. The team serves adults in the community who are unable to use traditional outpatient mental health services due to the seriousness and complexity of their problems.

The team members include: psychiatrists, psychiatric nurses, social workers, substance abuse specialists, counselors, job coaches, and a consumer advocate. Services include:

  • Mental health assessment and treatment
  • Psychiatric rehabilitation
  • Advocacy and outreach
  • Continuous care treatment
  • 24-hour, on-call coverage
  • Medication evaluation, prescription, and monitoring
  • Assistance with entitlements
  • Representative payee service
  • Nursing services
  • Dual diagnosis treatment
  • Assistance in securing housing

For Children

The child PACT team is a program intended to meet the mental health needs of children and adolescents, ages 6-17. The team serves children and adolescents in the community who are unable to use traditional outpatient clinic services due to the seriousness and complexity of their problems.

The team members include a child and adolescent psychiatrist, social workers, and counselors. Services include:

  • Mental health assessment and treatment to those for whom traditional outpatient mental health services have not been effective
  • Advocacy and outreach for parents and children
  • Coordination of services
  • 24-hour, on-call beeper coverage
  • Medication evaluations
