How do I sign up?
If you are not comfortable using a computer, it might be a good idea to ask a trusted family member or friend to help you. The enrollment process needs to be very specific to your information in order to grant you access to your records in a secure way.
We have developed the following step-by-step video (Click Here) to help you with the process. You can stop and replay the video as often as you need to understand the instructions.
In order to enroll, you need to have had a recent visit to our hospitals where your email was recorded at registration. Then, you will need the following:
- Date of Birth
- Medical Record Number
If we do not have your email address on file at UM UCH self-enrollment is not possible. You will need to contact the UM UCH Medical Records department in person to register your email. For security reasons, this is necessary so we can validate that you are the patient.
Your Medical Record Number (MR# or Med Rec # shown on sample below) can be found on the Portal enrollment instructions provided during your last visit to the hospital. When enrolling you must also use the email address you provided to us.

Once your enrollment is successful, you should put your personal user name and password in a safe place for future review and to keep your medical records secure.
