Memory Program
Forgetting someone’s name or what day of the week it is can be considered normal, but what about forgetting bigger things – like how to use the phone or where you live? How do you know when it’s forgetfulness or something more?
Our Memory Program is designed to help people with difficulties in thinking and memory. From mild impairment to early signs of dementia, we work with individuals and families to help them stay safe, independent and engaged in their regular, everyday activities.
Treatment
Our team of occupational and speech therapists can help reduce stress for both patients and caregivers and ensure patients continue to live as safely and independently as possible. We partner with patients and their families to create a personalized plan and connect them with tools and resources that can help.
Our program offers:
- Personalized goals and treatment plans
- Recommendations on remaining independent and safe at home
- Assessment of needs and evaluation of functional skills like memory and mobility
- Connections to helpful, local resources and support options, when needed
- Strategies and support to help improve memory and communication skills
While the work of improving or adapting belongs to the individual and their support system, our team is here to guide the process — through regular visits, goal-setting and expert support.
If you or a loved one has noticed changes in memory, mobility, balance or the ability to manage daily tasks, talk to your provider for a referral to our program.
