PLEASE READ: This registration form is designed to facilitate the scheduling of COVID-19 vaccination appointments for employees of the University of Maryland Medical System (UMMS). Due to limited supply, UMMS is allocating this vaccine among its workforce based on national prioritization frameworks. Because of this, it is critical that employees complete this registration if and only if you have been told by your manager and/or local leadership that you are eligible to receive the COVID-19 vaccine at this time.
