Call Out and Safe Return to Work

The Employee Call-Our Form is designed to streamline the call-out process during the COVID-19 pandemic. Employee Health will access submitted information and will give guidance on your safe return to work and directions for managing your health condition.

To notify Employee Health about your absence, please fill out this Employee Call-Out Form. 

If you do not have access to a computer or smartphone to submit your absence, you must call Employee Health at 410-448-6760 and leave a message. Please include the following details in your message: 

• The date
• Your name
• Your unit manager
• A reason for call out
• A phone number where we can reach you.
