Transform Health MD's Data and Analytics Team enhances the ability of practices and providers to make data-driven decisions when instituting care delivery changes by providing access to a rich data set comprised primarily of claims information.

Reports

Transform Health MD provides participating provider practices a range of data insights and reports for managed populations such as:

  • Attribution/Enrollment - Used to empanel the population to providers and care teams
  • Cost and Utilization - Provides insight into Per Member Per Month (PMPM) costs, emergency department (ED) visits, and acute inpatient stays across the managed population
  • Risk Stratification - Identification of at-risk patient populations based on a suite of proprietary risk stratification models targeting the right resources toward the right patients
  • Ad-hoc Reporting - Generated to examine specific trends or queries. Examples include skilled nursing facility (SNF), home health, and rehabilitation facility utilization as well as insight into patients with chronic conditions and specialist utilization

Analytics

Transform Health MD leverages extensive health care industry knowledge to provide recommendations for cost containment, identify trends in utilization, discover gaps in care opportunities, and compare provider performance to national trends and standards.

Health Information Exchange

The Transform Health MD Data Analytics Team also has a strong relationship with the Chesapeake Regional Information System for our Patients (CRISP), the state-designated Health Information Exchange (HIE) organization in Maryland. Our team can act as a liaison between providers and CRISP to ensure bi-directional connectivity and optimization of HIE services, when necessary.
