Transform Health MD's Pharmacy Team collaborates with practices, providers and the Multidisciplinary Care Team to provide clinical pharmacy support services. The Pharmacy Team works in the outpatient setting through various mediums (e.g., telephonic, telehealth, face-to-face) to work with the patient and/or caregiver to develop personalized care plans addressing medication concerns.

The Pharmacy Team also uses comprehensive medication therapy management (CMTM) as the standard of care when evaluating a patient's medication regimen to optimize medication therapy, improve adherence, and improve quality and cost.

Services

  • Comprehensive Medication Therapy Management (CMTM)
    The Pharmacy Team evaluates a patient's medications to identify drug interactions, adverse reactions, optimization opportunities and adherence concerns. The pharmacist works with the provider to suggest dosage adjustments, therapy and potential opportunities for deprescribing, all with the aim of improving the patient's medication-based treatment regimen.
  • Medication Adherence Support
    The Pharmacy Team helps patients identify adherence barriers such as costs, side effects, complex drug regimens and cognitive barriers. The team also works with the Health Care Team to adjust medications when appropriate and/or connect the patient to adherence resources such as medication delivery and bubblepackaging services.
  • Transitions of Care Services
    After transition of care, the Pharmacy Team will conduct a medication review with the patient/caregiver to establish an updated medication list. The list will be carefully evaluated for any gaps in therapy or medication-related problems. The pharmacist discusses with the provider recommendations for medication reconciliation. 
  • Education
    The Pharmacy Team is available to provide medication, disease state and Medicare Part D education to patients and caregivers.
  • Pharmacotherapy Consults
    Pharmacists are available to the provider and practice for consults on medication-related issues such as kidney and liver dose adjustments, Beers Criteria medications, high-risk medications (such as insulin and warfarin), and drug interactions.
