The Pharmacy team collaborates with practices, providers and the multidisciplinary care team to provide clinical pharmacy support services. The pharmacy team works in the outpatient setting through various mediums (telephonic, telehealth, face-to-face) to develop personalized care plans with the patient and/or caregiver to address their medication concerns.
The team uses comprehensive medication therapy management (CMTM) as the standard of care when evaluating a patient's medication regimen to optimize medication therapy, improve adherence and improve quality and cost.
- Comprehensive Medication Therapy Management (CMTM)
The pharmacy team will assess and evaluate a patient's medications (prescription and OTC) and vitamins/supplements for drug related problems, of which may include; condition-drug interactions, drug-drug interactions, adverse effects and medication compliance.
- Medication Adherence Support
The Pharmacy Team assists patients in identifying reasons for not taking medications as prescribed and work with the health care team to adjust medication regimens when appropriate and/or connect the patient with resources to assist with overcoming those barriers.
- Transitions of Care Services
Following a transition of care, the pharmacy team will review medications with the patient/caregiver to identify a most up-to-date medication list. This medication list will be evaluated for gaps in therapy and the need for medication adjustments that may have occurred around a transition with recommendations made back to the providers.
The pharmacy team is available to provide medication, disease state and Part D formulary/plan structure education to patients and their caregivers.
- Pharmacotherapy Consults
Pharmacists are available to the provider and practice for consults on medication related issues such as renal/hepatic-dose adjustments and drug interactions
