Employee COVID-19 Vaccination Reporting
For UMMS employees and members of the UM School of Medicine clinical faculty only
If you've previously submitted COVID-19 vaccine documentation to UMMS or received your primary series or prior bivalent boosters from UMMS Employee Health:
- Did you receive the latest COVID-19 booster that was made available on or after September 12, 2023?
- If "yes," please submit documentation now.
If you've never submitted COVID-19 vaccine documentation to UMMS and did not receive your primary series or bivalent booster doses from UMMS Employee Health:
- Are you a recently hired employee who has never submitted COVID-19 vaccine documentation?
- If "yes," please submit documentation now.
After You Submit Your Documentation
- After submitting your documentation, you'll receive an email (to the email address you provided on the form) confirming Employee Health's receipt of your documentation.
- It may take up to two weeks for you to receive confirmation that the review of your documentation is complete. We ask that you do not call your local Employee Health department to confirm the status of your documentation. If Employee Health has questions, they'll reach out to you directly.
- Please do not submit your documentation more than once.
