Due to COVID-19 (Coronavirus) we have made changes to the process for requesting medical records for patients who received care at any UM Capital Region Health medical entity.

We are no longer able to accept in-person or walk-in requests.

We can only accept requests that are mailed in by the patient or faxed by a healthcare provider (primary care provider, specialist provider or a medical facility such as an urgent care, emergency room or hospital).

This change applies to all UM Capital Region Health entities:

  • UM Prince George's Hospital Center
  • UM Bowie Health Center
  • UM Laurel Medical Center
  • UM Capital Region Surgery Center
  • UM Capital Region Health Medical Group (all practices and locations)

Requesting Medical Records

The information contained in the patient's medical record is confidential. We require authorization from the patient or guardian in order to release medical records. To request medical records for services rendered at any of the entities, please download the Authorization for Release of Protected Health Information form and mail the completed form to:

If you are the legal guardian, healthcare agent, power of attorney or other legal representative of a patient, you must also attach proof of your authority to act on behalf of the patient.

We are able to accept and honor fax requests from a medical provider on behalf of any patient as part of our commitment to continuity of care. Providers should fax the medical request form to 301-618-2997

Fees

There is a fee to copy medical records unless records are being requested for continuity of care. The charge is 76 cents a page, plus a $22.88 preparation charge.

There is no charge for records sent directly to a physician or healthcare facility for continuing care. The fees comply with applicable Maryland State guidelines.
