If you need a copy of your medical records. The Health Information Management (HIM) Services team at University of Maryland Capital Region Health has an easy process in place to help you get the information you need.

The first step is to complete an Authorization for Release of Protected Health Information form.

Be sure to complete all sections of the document, including the signature and date, so we can quickly complete your request.

If you are the legal guardian, healthcare agent, power of attorney or other legal representative of a patient, you must also attach proof of your authority to act on behalf of the patient.

You can submit the information release form by mail, in person or by fax, in urgent situations. We will only fax medical records to another healthcare provider or facility for continuing medical care. We can mail the copies to the address you specify on the authorization form, or you can arrange to pick them up during our hours of operation.


Yes, there is a fee to copy medical records. The charge is 76 cents a page, plus a $22.88 preparation charge. (This fee is not charged to patients.)

There is no charge for records sent directly to a physician or healthcare facility for continuing care. The fees comply with applicable Maryland State guidelines.

Once you complete the authorization form, you can mail it, present it in person or fax it to the appropriate University of Maryland Capital Region Health facility. Please note, we only accept fax submissions for urgent, continuity of care requests to physician offices or other urgent care settings.

If you were born in Maryland, you can obtain a copy of your birth certificate from the Division of Vital Records of the Maryland Department of Health and Mental Hygiene.

The division also issues copies of other vital records and events that occurred in Maryland, including:

  • Death certificates
  • Fetal death certificates
  • Marriage certificates
