After a hospital stay, you might need extra support to help you adjust to being at home. Our transitional care clinic helps you manage your care and stay healthy. Created by our Population Health team and the University of Maryland School of Pharmacy, the clinic pairs patients with a nurse practitioner, social worker, a pharmacist and representatives from the University of Maryland School of Law.
We know many kinds of stress can affect your health. Our goal is to address problems that may affect your well-being. We provide access to:
- Nurses who can answer your health questions and coordinate follow-up appointments
- Pharmacists who can help you get the medicines you need and ensure you take them correctly
- Attorneys who can address issues causing stress at home, such as a disagreement with a landlord
- Social workers who can address the social determinants of health and link you to community programs to meet your needs
