Individuals who have recently been diagnosed with prediabetes or are at risk for Type 2 diabetes, are invited to enroll in the Diabetes Prevention Program. This free, evidence-based program helps people with prediabetes reduce their risk of developing Type 2 diabetes and improve their overall health.

This one-year program consists of 16 weekly group sessions followed by 7 monthly maintenance sessions. Participants gain access to a lifestyle coach that is specially trained to lead the program as well as a support group of individuals with similar goals and challenges. They will learn how to:

  • Eat healthy
  • Be active
  • Self monitor
  • Problem solve
  • Manage stress
  • Make lifestyle changes
  • Stay motivated
  • Be positive

Program Eligibility

To be eligible for the Diabetes Prevention Program, participants must:
  • Be at least 18 years old and
  • Body Mass Index ≥25 (≥23 if Asian) and
  • Have no previous diagnosis of type 1 or type 2 diabetes and
  • Have a blood test result in the prediabetes range within the past year:
    • Hemoglobin A1C: 5.7–6.4% or
    • Fasting plasma glucose: 100–125 mg/dL or
    • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL or
    • Take the Prediabetes Risk Test and receive a screening result of high risk for type 2 diabetes.

Upcoming Program Cohorts

The program will offer both virtual and in-person cohorts. Once enrolled, you will receive a full schedule of class dates. You will be able to select your desired cohort when completing the Risk Assessment Form.

  • Virtual - Group A
    Group A will meet on Tuesdays from 12 - 1 p.m. beginning January 28, 2025.
  • Virtual - Group B
    Group B will meet on Thursdays from 5 - 6 p.m. beginning February 27, 2025.
